Post Hospital Discharge Program

HMSA’s Post Hospital Discharge Program supports your goal to maintain or reach your optimal health after a hospitalization or emergency room visit. We work closely with your doctors to help coordinate your care, including your treatment, medications, and other services. We want to make sure that you have the follow-up care you need.

This program is voluntary and available at no cost to eligible HMSA members.

How will I work with my care manager?

You have access to a team of Hawaii-based transitional care nurses who connect with you or your caregiver by phone, virtual visit, or in person. You can meet or talk with them from your home, in a provider’s office, or at an HMSA Center.

You’ll receive:

  • Help to make sure that you understand your discharge instructions.
  • Coordinated care with your primary care provider (PCP) or specialists. For example, your care manager will make sure you have a follow-up appointment with your PCP within one week after you’re discharged from the hospital.
  • Help with understanding your medications and treatment plan.
  • An action plan with steps to take in case you experience worsening symptoms related to your condition.
  • Help to set and achieve health goals.
  • Information about community resources to support your health and well-being.
  • Discussion of advance care planning (e.g., advanced health care directive, living will, or power of attorney).

Mail Service Order Form
Mail-order prescription medications are available to most HMSA drug plan members. You can save money with mail-order drugs because copayments are generally lower than when you buy from retail pharmacies. Check your drug plan certificate to see if you have mail order as a benefit.
