Programs

• Primary Care Payment Model is HMSA’s effort to change provider compensation to ensure the sustainability of independent providers in a free-choice system. This replaces Pay-for-Quality and Patient-centered Medical Home (PCMH) programs.
• Federally Qualified Health Centers (FQHC) and Rural Health Clinics (RHC) Federally Qualified Health Centers and Rural Health Centers can participate in a unique Pay-For-Quality program that aligns incentives for the provider.

Care Programs

• Disease Management Services promote optimal health for patients through support of patient-physician interaction and ongoing patient education on diabetes, asthma, chronic kidney disease, COPD, and cardiovascular disease.
• Medicare Star Ratings support the efforts of CMS to improve the level of accountability for the care provided by physicians, hospitals, and other providers.
