Basic Claims Filing - The CMS 1500 Claim Form

HMSA's Basic Claims Filing training module can be used to train new office staff or as refresher training for experienced office staff. HMSA encourages providers and their staffs to quiz themselves on their knowledge of HMSA claims filing.

Although the training documents refer to the CMS 1500 claim form, the training can also be valuable to providers who submit claims by electronic data interchange (EDI), as the basic principles regarding required information remain the same. In addition, many providers who use EDI, also submit CMS 1500 claims in certain situations.

Remember, a simple error, such as a transposition or a check mark in the wrong box, can cause your claim to be delayed or denied. Attention to the small details is key to successful claims filing.

Take the Quiz

There are 20 questions in the quiz. If you miss questions, we encourage you to review the relevant documents again. Upon completion of a quiz, click on the Check Answers button and an answer page will appear with the participant's score. If wrong answers are identified, the participant will see a narrative that reveals and explains the correct answer. The answer page can be printed as a record of employee training. If a number of questions are missed, the student review the relevant documents and repeat the quiz. The quiz may be taken as many times as necessary to attain proficiency.

Results can be evaluated as follows:

19 to 20 questions correct = proficient
18 questions correct = good understanding
17 questions correct = fair understanding
16 or fewer questions correct = poor understanding; high likelihood of claims being returned, delayed or denied
