The Federal Employee Program (FEP) provides outpatient professional care benefits for services billed as diabetic education programs by covered professional providers, when the patient has a diagnosis of diabetes.
FEP provides education for diabetic education provided by licensed medical practitioners acting within the scope of their licensure under applicable state laws. The FEP PSHB program membership cards are identified by coverage codes 33D, 33E, and 33F for the FEP Blue Standard Option and 33A, 33B, and 33C for the FEP Blue Basic Option. FEP Blue Focus enrollment codes are 35A, 35B, and 35C.

**When billed for by the outpatient department of a hospital, outpatient medical care benefits apply.
