Change of Ownership, Name, Service Sites, Bankruptcy

The participating pharmacy must notify HMSA of:

• Intention to transfer ownership or change of name at least 60 days in advance of the change.
• Elimination of any service sites at least 30 days in advance.
• Insolvency, a general assignment for the benefit of creditors, voluntary petition for bankruptcy, reorganization, adjudication or appointment of a receiver, trustee or custodian for any substantial portion of its properties in assets not more than five business days after the filing or the pharmacy’s receipt of notice, whichever occurs first.
