Community Integration Services Plus (CIS+) – previously named Community Integration Services (CIS) — assists eligible QUEST members with becoming fully integrated members of the community as well as achieving improved health outcomes and life satisfaction. Effective January 1, 2026, the CIS+ program includes two types of housing-related benefits: housing navigation supports and medical respite. The CIS+ program encompasses two categories of benefits: housing navigation supports include both pre-tenancy support and tenancy sustaining services, while medical respite includes short-term pre-procedure housing, short-term recuperative care, and short-term post-hospitalization housing. Medical Respite is limited to 6 months aggregate per rolling 12-month period.
To be eligible for CIS+, QUEST members must meet both social and clinical eligibility requirements. Table 1 outlines the benefits available as of January 1, 2026. These criteria may be updated as MQD gradually expands eligibility to additional populations. Further clarification on eligibility can be found here.

Any QUEST Integration (QI) eligible member who meets eligibility criteria can be referred to HMSA for CIS+ screening to determine program eligibility. There are no restrictions on who can make the referral. The referral form and consent form can be faxed to 808-948-8243 or emailed to HMOps-HCCIS@HMSA.com. Please note that Medical Respite Authorization Forms signed by qualified clinicians (MD, DO, NP, PA) is also required for eligibility/authorization of medical respite services.
For housing and tenancy services, QUEST members enrolled in Community Care Services (CCS) behavioral health organization (BHO) will receive those benefits through the CCS plan. For medical respite, whichever entity receives the referral will be responsible for responding to the request, determining eligibility, and administering the benefit for the initial authorization period. After the initial authorization period, administration of the benefit and additional authorizations will be conducted by the CCS plan.

You will be able to access the online application using the link below or through our HHIN+ login in page by going to the new Apply to HMSA link in the footer. Access to HHIN+ is not required to complete the application.
