Diabetes Prevention

Do you have patients who are at risk for diabetes?

They may be eligible to enroll in the Medicare Diabetes Prevention Program (MDPP). Patients in the program can reduce their risk of diabetes through lifestyle support and coaching for up to 12 months at no cost (once per lifetime).

Who’s eligible?

Your HMSA Akamai Advantage® patients who meet all of the following criteria:

  • Met 1 of 3 blood test requirements within 12 months of their first session:
    • Hemoglobin A1c test with value between 5.7% and 6.4%
    • Fasting plasma glucose test of 110-125 mg/dL
    • 2-hour plasma glucose test (oral glucose tolerance test) of 140-199 mg/dL
  • Body Mass Index (BMI) of at least 25 (23 if patient self-identifies as Asian)
  • No history of type 1 or type 2 diabetes
  • Never been diagnosed with ESRD
  • No previous MDPP services
