Claims Filing Highlights – IV Therapy

• Bill a range of dates only when services are rendered on each day of the range. For example, if the range of dates is from July 1 to July 5, HMSA will assume that services were rendered on July 1, 2, 3, 4 and 5. The number of services in the days/units column would be five.
• Do not bill services for overlapping months on the same claims form. For example, do not include a date range from July 15 to August 11. In such a case, separate the claim into two claims, with the first claim for services rendered from July 15 to 31, and the second claim for services from August 1 to 8.
• Do not bill for future dates. For example, if you are billing for services rendered from July 15 to July 31, you may not submit the claim on July 23. Wait until the services you are billing for have actually occurred before submitting a claim for those services.
