Effective July 1, 2026, the AccordantCare® management program will only be available to retired state employees under the Hawaii Employer-Union Health Benefits Trust Fund (EUTF).
The AccordantCare™ program is a CVS Health program that gives EUTF members support to understand and manage one or more of the twenty-one medical conditions included in the program:
- Amyotrophic lateral sclerosis
- Chronic inflammatory demyelinating polyradiculoneuropathy
- Crohn’s disease
- Cystic fibrosis
- Dermatomyositis
- Epilepsy (seizures)
- Gaucher disease
- Hemophilia
- Hereditary angioedema
- Human immunodeficiency virus
- Inclusion body myositis
- Juvenile idiopathic arthritis
- Multiple sclerosis
- Myasthenia gravis
- Parkinson’s disease
- Polymyositis
- Rheumatoid arthritis
- Scleroderma
- Sickle cell disease
- Systemic lupus erythematosus
- Ulcerative colitis
If your EUTF retired patient chooses to enroll in the program, they will be assigned to an AccordantCare nurse who will conduct periodic assessments and other interactions either by phone or online. The nurse will work closely with you and your staff to help ensure that the patient understands your instructions, follows your treatment plan, and makes appropriate progress. Your patient may also be assigned a resource specialist or case manager as needed for additional help. There is no extra cost to your patient for this service.
Who is eligible for the program?
EUTF Retired state employees with any of the 21 conditions listed above are eligible to participate.
