Filing Medical Claims for Services from Nonparticipating Providers

Participating providers in Hawaii file claims to HMSA for your care. Nonparticipating providers can also file claims to HMSA. (To check if your provider participates with HMSA, click here.)

If your nonparticipating provider in Hawaii or out of state doesn’t file a claim for you, you can submit your reimbursement request to us.

Important Notes: 

  • Reimbursement requests must be received within a year from the last day that services were received. For HMSA’s Plan for Federal Employees, reimbursement requests will be accepted until Dec. 31 of the year after the year services were received. Refer to the federal plan brochure for more information.
  • Processing of your claim may take at least 90 calendar days or longer depending on the complexity of the claim, the volume of claims HMSA is processing, or if the claim is missing essential information.

You may submit your reimbursement request by doing one of the following:

  • Submit a claim online (this recommended option typically results in the quickest processing time):
    • For services received from out-of-state providers, send HMSA a Medical Receipts Form and the necessary documentation using My Account.
    • For services received from out-of-country providers, use the International Claim Form and follow the instructions on the form.
  • Mail a printed claim:
    • For services received from out-of-state providers, complete the fillable PDF form [PDF], print it, and mail the claim with the necessary documentation to the address indicated on the form.
    • For services received from out-of-country providers, complete the International Claim Form, print it, and mail the claim with the necessary documentation to the address indicated on the form.
    • You can also contact HMSA to mail a form to you, or pick one up at any of our HMSA Center locations.

Copies of the information and any supporting documents you send to HMSA should be clear and legible, with your HMSA subscriber number written on each page. Please keep the originals for your records. The documents you submit to HMSA won’t be returned to you. Any missing information could delay processing of your request.
