Consolidated Appropriations Act Certification – FAQ

  1. When I report my information, which employees do I include?
    Please include all full-time, part-time, and seasonal employees, and group-sponsored COBRA members who contributed monthly premiums to your HMSA plan in 2025.
  2. What if an employee was terminated mid-year or after only a few months?
    Please include all employees who contributed to your HMSA plan 2025 premiums, even if they were employed only part of the year.
  3. In the past, you requested member totals to calculate a per-member, per-month average. Do I need to include that information this year?
    No. Before the 2023 reference year, average premium was calculated on a per-member, per-month basis. To simplify the calculation, starting with plan year 2023, the monthly average is no longer on a per-member basis. You can divide your annual total premium amounts (member and employer) by 12 to get the monthly average.
  4. Can I combine the premium amounts paid for all health plan carriers?
    No. We only need premium amounts for the HMSA-covered employees and dependents for 2025.
  5. Where can I find the information on premiums paid to HMSA for 2025. Can you provide me that information?
    The information can be found on your billing statements.
  6. My company has a few subsidiaries and numerous plans with HMSA. How should I report the requested information?
    Please work with your account executive to report data for multiple groups using one submission.
  7. Can I estimate the premium amounts, or do I need to provide exact amounts?
    We’re required to report exact amounts.
  8. How can I get confirmation that you’ve received my information?
    A copy of the receipt will be posted here once available. If you have questions about your submission, please contact your HMSA representative.
  9. What if I don’t provide HMSA with the requested information?
    If we don’t receive your information in a timely manner, HMSA won’t include your premium share information in our report.
