Your choice of HMO Options
This plan offers two HMO
options that feature a low, annual deductible to fit your
budget:
- High Option is $300 per person, or $900 per family contract.
- Basic Option is $500 per person, or $1,500 per family contract.
Large provider network
Through HMSA’s network
of 1,800 medical providers, you and your eligible dependents
can select a health center
and a personal care provider (PCP) to coordinate your
care.
Predictable costs
This plan offers you the convenience of knowing your out-of-pocket costs before
you see your doctor. Under HMSA’s Individual Care Plan, it’s easy to
manage your medical finances with:
- A fixed $20 copayment
for physician office visits and health evaluations.
- 100-percent coverage for physical exams and preventive services.
- Low annual deductibles for both plan options.
Coverage away from home
Rest assured that when you are traveling on the Mainland or Neighbor Islands, you
have access to the nation’s largest provider network through the Blue Cross
and Blue Shield network of providers.
Protecting your family
Under this plan, your qualified dependents may also be covered under this plan,
maintaining their good health through preventive services.
