Disclosure Statement

In order to be an HMSA participating provider, all providers must pass:

  • HMSA’s initial credentialing process: Providers must submit their information to HMSA.
  • HMSA’s re-credentialing process: Providers are asked to review and update any information every three years.

Providers are expected to notify HMSA of any changes as soon as possible.

There may be constraints because providers and hospitals supply their own data to HMSA. It is important you know what is verified and what is not.

Physician

Name and Gender

Limitation: The provider gives us the info.

Specialty

The provider’s area of practice, such as branch of medicine, procedures, age range of patients, body systems, or types of diseases.

Limitations: None. HMSA verifies this.

Hospital Affiliations and Privileges

The physician includes the hospitals he or she is approved to admit and care for patients.

Limitations: The provider gives us the info.

Health Centers

A group of physicians from which all health care services are provided or arranged.

Limitation: The provider gives us the info.

Board Certified

Provider’s area of training or area in which they have passed a medical specialty examination.

  • American Board of Medical Specialties (www.abms.org)
  • American Medical Association (www.ama-assn.org)
  • American Osteopathic Association (www.osteopathic.org).

Limitations: None. HMSA verifies all certifications.

Accepting New Patients

The provider lets us know if he or she is able to take on new patients.

Limitations: The provider gives us the info.

Language

The provider lets us know if he or she speaks a language other than English.

Limitations: The provider gives us the info.

Accreditation

  • A hospital is certified by an accredited board, such as The Joint Commission (www.jointcommission.org) and the Commission on Accreditation of Rehabilitation Facilities (www.carf.org), to meet the board’s quality standards.
