Because 65C Plus is a Medicare-based plan, the Local Coverage Determination (LCD) policy decisions for Hawaii documented on this site apply to 65C Plus.

 

Effective October 1, 2006, NAS will become the Durable Medical Equipment Medicare Administrative Contractor for Jurisdiction D, which includes Hawaii. A schedule of communications explaining the transition process can be accessed from the link above.

 

  2007 Fee Schedule can be accessed from the NAS website.
