Your HMSA membership card identifies you as an HMSA member and enables providers to determine your plan benefits. Always carry your card with you and present it whenever you receive services. This will help to ensure your claims are processed correctly.
If you lose your card, My Account on hmsa.com provides the most convenient way to request a new one. Just log in to My Account and select Request ID Card.
New HMSA membership card
You’ll receive a new card when your plan information changes. Your covered spouse and dependents will also receive new cards.
Examples of changes that affect the information on your HMSA membership card:
- Your medical plan has changed.
- The dependents on your HMO plan have changed.
- Your employer discontinues drug coverage through HMSA.
