You may request duplicate HMSA membership cards online. Just log onto My Account, hover over Profile, and click Request card. If you need your subscriber ID to register for My Account, please call HMSA Customer Relations for help. You may also request your duplicate card when you call.
You’ll receive a duplicate card in five to seven business days.
You’ll receive the same number of cards previously provided to you, unless:
- You canceled or added a dependent.
- A dependent turns 18.
If a card is urgently needed, please call HMSA Customer Relations to request a temporary card.
