HealthPass

When is the last time you took an inventory of your health and lifestyle? Not just health statistics like blood pressure and cholesterol, but an in-depth look at the way you live each and every day. A HealthPass screening is an opportunity for you to step back and assess your lifestyle.

You’ll start by completing a Succeed health risk assessment questionnaire. This will help you prioritize what kinds of health changes you may be ready to make, based not only on your risk factors, but also on your readiness and confidence to make a change.

Next we’ll take your physical measurements, including blood pressure, height and weight, body mass index (BMI), blood cholesterol, blood sugar, and do a waist circumference and body fat analysis. These measurements help indicate your risks for cardiovascular disease and diabetes. Depending on your age, gender and risk factors, you may be eligible for other screenings, such as a bone density test for osteoporosis or a sigmoidoscopy to check for colorectal cancer.

Finally, we’ll put the two together. A professional health consultant will help you determine your health risks, which lifestyle habits are having the most impact on your health, and what you can do to improve your health.

After you complete the screening, you can use HealthMedia interactive lifestyle programs online at home to meet your lifestyle goals. Program choices include weight management, nutrition improvement, stress management and smoking cessation.

If you would like to complete the lifestyle program on paper, please let the health consultant know at the time of your HealthPass screening.
