HealthPass Closing
Learn about our exciting new Well-Being Connection program, which will replace HealthPass in 2012. For more information, call (808) 948-6079 on Oahu or 1 (800) 776-4672 toll-free on the Neighbor Islands.
When was the last time you took an inventory of your health and lifestyle? Not just
health statistics like blood pressure and cholesterol, but an in-depth look at the
way you live each and every day. A HealthPass screening is an opportunity for you
to step back and assess your lifestyle.
You’ll start by completing a health risk assessment questionnaire. This will
help you prioritize what kinds of health changes you should make based
on your risk factors, and your readiness and confidence to change.
Next, we’ll take your physical measurements, including blood pressure, height
and weight, body mass index (BMI), blood cholesterol, blood sugar, waist circumference,
and body fat. These measurements help indicate your risks for cardiovascular disease
and diabetes. Depending on your age, gender and risk factors, you may be eligible
for other preventive exams offered through HealthPass.
Finally, we’ll put the two together. A professional health consultant will
help you determine your health risks, which lifestyle habits are having the most
impact on your health, and what you can do to improve your health.
If you would like to complete the lifestyle program on paper, please let the health
consultant know during your HealthPass screening.
