The plan also includes a $15,000 group term life and accidental death
and dismemberment benefits policy.
- Unlimited benefit protection: To protect you against serious
illness or injury, this plan provides unlimited benefits for covered
expenses.
- Precertification: This plan includes features that control
expenses by helping you get appropriate health care in the proper
setting. Before services are provided, benefits for nonparticipating
or out-of-state hospital admissions, certain diagnostic tests, and
certain surgeries must be approved.
- Financial protection: This plan has a maximum annual copayment
of $2,500, which includes the annual deductibles per calendar year.
If this maximum is reached, you will owe no copayment for covered
services for the rest of that calendar year.
