Plan 6 is an HMSA Preferred Provider Plan for individuals. The plan includes office
visits, inpatient and outpatient facility services, and other provider services.
To get the most from Plan 6, you should seek services from HMSA participating providers.
This plan includes different categories of coverage:
- With single coverage, the subscriber is the only one covered.
- With two-party coverage, the subscriber and one eligible dependent, such as your
spouse or a child, are covered.
- With family coverage, the subscriber, spouse and each eligible dependent are covered.
