can select a health center and a personal care physician (PCP) to coordinate your
care.

This plan offers you the convenience of knowing your out-of-pocket costs before

- A fixed $17 copayment for physician office visits, well-child visits, and health
evaluations.
- 100-percent coverage for physical exams and preventive services.
- Low, annual deductibles for both plan options.
Coverage away from home
Rest assured that when you are traveling on the Mainland or Neighbor Islands, you
have access to the nation’s largest provider network through the Blue Cross
and Blue Shield network of providers.
Protecting your family
Under this plan, your qualified dependents may also be covered under this plan,
maintaining their good health through preventive services.
