The Health Plan Hawaii (HPH) Conversion Plan is a health maintenance organization (HMO) plan that provides access to health care for you and your eligible dependents after you leave HPH or HMO Hawaii group coverage.

Continuous coverage

Membership is guaranteed and coverage begins the day after your group HMO Hawaii or HPH plan ends.

Staying healthy

Keeping you healthy is at the core of HMSA’s HPH Conversion Plan. That’s why all routine preventive services are provided at no cost, allowing you to maintain good health to protect yourself against illness.

Keeping your physicians

Not having to switch doctors when you change plans is a big convenience. You and your dependents can keep the same primary care physicians and health centers.

Financial protection

A major benefit of the HPH Conversion Plan is the low out-of-pocket costs. You’ll have:

  • A $15 copayment for doctor office visits, allowing you to have predictable health care costs.
  • Once you reach the annual maximum copayment amount, the plan pays 100 percent of basic benefits for the rest of the calendar year.
