Comprehensive Coverage

Your HMO medical plan provides benefits for one routine vision exam per calendar year. Your supplemental vision care benefits cover services to identify and correct visual problems with lenses and other equipment.

Freedom of Choice

Eye exams are given by vision providers within your HMO health center. For a list of participating vision providers, consult your Health Plan Hawaii Directory available from your employer or Find a Doctor.

You will also receive benefits if you use a nonparticipating provider. The benefits, however, differ from those you would receive if you went to a participating provider.

Cost Savings

HMSA has contracted discounted rates with:

• Ophthalmologists.
• Optometrists.
• Opticians.
• Optical dispensers.

Eligibility: HMO vision benefits are available through HMSA’s HPH medical plan from your employer. Check with your employer regarding your health plan benefits.
