HealthPass’s SucceedTM is a resource that strives to improve your employees’ health.
This health risk assessment will evaluate their health, identify their health risks,
and show them what they can do to become healthier.
The first step in this health risk assessment is for your employees to complete
the Succeed online health risk assessment questionnaire. If your employees
prefer to complete the questionnaire on paper, simply inform our HealthPass staff.
After they have completed the online health risk questionnaire, the Succeed program
will analyze the information and generate a wellness action plan that is specific
to your employees’ needs and goals.
Your employees will also be given access to many available tools and resources to
further help them. These options are benefits of eligible HMSA plans and are at
no additional cost. Resources include:
- A HealthPass biometric screening and consultation.
- Preventive examinations.
- Health education classes.
- Clinical intervention programs on:
- Weight management.
- Nutrition.
- Smoking cessation.
- Stress management.
- go Get on Track (an online physical activity tracking
program).
- Physical fitness assessments.
- Telephone consultations and health coaching.
They will also have access to HealthMedia’s® interactive lifestyle programs that
can be used in the privacy of their home.
After completing their health risk questionnaire, your eligible employees can schedule
an appointment for their HealthPass biometric screening. For added convenience,
HealthPass can come to your worksite to conduct these screenings. We will take their
physical measurements including their blood pressure, height, weight, body mass
index, blood cholesterol, and blood sugar, and do a body fat analysis.
A professional health consultant will then go over their results and personal plan
with them, answer their questions, and discuss lifestyle changes they would like
to make. These measurements will help indicate their risk for cardiovascular disease
and diabetes. The results can also be used in conjunction with mapping their personal
goals.
