Disease Management
Chronic conditions are a major driver of health care costs today and result in decreased
productivity and increased absenteeism. You can help improve the health of your workforce
by encouraging employees to take advantage of programs and services included in their HMSA
plan at no additional cost.
HMSA’s Care Connection is a disease management program for members with chronic health
conditions. The program can help your employees enjoy an active life with fewer symptoms,
meet their health care and dietary goals established by their doctor, and use information,
classes and tools to manage their condition.
Our specially trained Care Connection staff provides information and support for:
- Asthma and chronic obstructive pulmonary disease: Helps employees understand their condition, triggers and symptoms better.
- Behavioral health: Helps employees manage and overcome stress, anxiety, depression, substance abuse, smoking, or other behavioral issues.
- Heart failure and coronary artery disease: Helps employees manage their heart condition, medications and diet.
- Diabetes: Helps employees manage and control diabetes to have fewer symptoms and avoid long-term problems.
- Chronic kidney disease.
HMSA’s Care Connection program does not replace a doctor’s care. It helps your employees
better understand their condition and support their doctor’s treatment plan. Employees
enrolled in Care Connection will receive:
- A welcome packet and workbook.
- Quarterly newsletters.
- Health education specific to their needs.
- Reminders for tests and/or exams.
Care Connection staff will call your employees and explain how the program works.
