Your health center
It's important for HMO members to choose a health center and a personal care provider (PCP).
A health center can be a traditional clinic or it can refer to a group of affiliated providers who practice at various locations.
Each HMO plan member can select their own health center and PCP. The PCP coordinates care and refers patients to appropriate medical specialists within the health center.
Use HMSA's Find a Doctor tool or call HMSA Customer Service for a list of health centers and associated providers.
