What is a lifetime maximum?
A lifetime maximum is the maximum dollar amount that will be paid on your behalf under your HMSA plan. See your Guide to Benefits for more information.
To find the maximum dollar amount HMSA will pay for benefits under your plan or how much you have accumulated towards your lifetime maximum, log in to My Account on hmsa.com and click the View Benefits link on the right of the home page, or contact HMSA Customer Relations.
Renewal provision
If you reach your lifetime maximum, HMSA will cover medical expenses up to $10,000 per calendar year. This is called a lifetime renewal.
- The lifetime renewal begins Jan. 1 of the year following the year you reached your lifetime maximum.
- The $10,000 will renew Jan. 1 of each year as long as you remain covered under the plan.
- Unused lifetime renewal benefits not used in one calendar year do not carry over to subsequent years.
If you exhaust both the lifetime maximum and the lifetime renewal amount in any given calendar year, you are not eligible for benefits for any covered services until the following calendar year.
Note: The renewal provision is applicable only to the dollar lifetime maximum. Covered services limited by frequency lifetime maximums, such as in vitro fertilization, are not subject to a renewal provision.
