Request duplicate HMSA cards
You may request duplicate HMSA membership cards through My Account on hmsa.com. Just log in to My Account and click the "Request ID Card" link on the right of the screen. If you need your subscriber ID to register for My Account, please call HMSA Customer Relations for assistance. You may also request your duplicate card when you call.
You will receive a duplicate card in 5 to 7 business days.
You will receive the same number of cards previously provided to you, unless:
- You cancelled or added a dependent.
- A dependent turns 18.
If a card is urgently needed, please call HMSA Customer Relations to request a temporary card.
