How to update your address with HMSA

We appreciate you letting us know when your address changes. Click the link below that matches your plan type for instructions.  

If you don’t see your plan or if you are not sure of the type of plan you have, please call HMSA Customer Service and we will assist you.  

Note: Only the subscriber on an HMSA plan or the subscriber's authorized representative may change an address on an HMSA plan.  Authorized representatives include:

  • Any person you authorize to act on your behalf by submitting the Authorized Representative Form to HMSA.
  • A court-appointed guardian or an agent under a health care proxy.

Members who have an HMSA plan through an employer

Some employers or groups do not allow you to report address changes to HMSA, so please check with them first. 

If your employer or group allows you to update your address directly with HMSA:

  • Update your address on My Account. My Account gives you access to certain plan information and allows you to perform certain online requests.
  • Mail your request to Membership Service, P.O. Box 860, Honolulu, HI 96808.
  • Call an HMSA customer service representative.

Note: Mailed requests must include your name, subscriber number from your HMSA membership card, your old and new addresses, and your signature. 

 

EUTF plan members

If you are an EUTF plan member who is currently employed, submit your request to your department’s personnel officer.

If you are an EUTF plan member who is retired, submit your request to EUTF:

  • Call 586-7390 on Oahu or 1 (800) 295-0089 toll-free.
  • Request the change in person at City Financial Tower, 201 Merchant St., #1520, in Downtown Honolulu.
  • Mail your request to Employer-Union Trust Fund, P.O. Box 2121, Honolulu, HI 96805-2121. The request must include your name, old and new addresses, Social Security number, and signature.

 

65C Plus (Cost) and Akamai Advantage members

Submit your request in writing via mail or fax to:

Honolulu, HI 96808

Fax: (808) 948-6614 on Oahu

The request must include:

  • Your name.
  • Subscriber number from your HMSA membership card.
  • Old and new addresses.
  • Your signature.

 

Individual plan members

Submit your request to HMSA:

  • Call an HMSA customer service representative.
