How to get copies of the drug claim form

The drug claim form is available to you in a variety of ways. You can print the Coordination of Benefits/Direct Claim Form from this article or from My Account on hmsa.com. You can also request copies by mail or in person.

To print the claim form online from My Account:

  • Log in to My Account.
  • Select Drug Tools from the menu on the left, then select the Forms & Cards link.
  • Scroll down to the bottom of the page and find the Claim forms for retail pharmacy purchases section.
  • Select the link labeled “Save time by printing the retail prescription claim form online.”

To request the claim form be mailed to you:

  • Log in to My Account and follow the steps above to get to the Claim forms for retail pharmacy purchases section. Select the link labeled "Request claim form," or
  • Call HMSA Customer Service and select the drug option to speak with a representative to request copies.

To request the claim form in person:
