HIPAA Certificate of Creditable Coverage
You will receive a Certificate of Creditable Coverage to document you were covered by an HMSA plan.
You may be able to use this certificate to reduce waiting periods for pre-existing conditions if your new health plan requires a wait.
HMSA will give you a certificate when:
- You lose HMSA coverage.
- You are entitled to elect HMSA COBRA continuation coverage.
- You lose HMSA COBRA continuation coverage.
- Your HMSA subscriber number or HMSA medical plan has changed.
- You request a certificate. Your request must be received by HMSA within 24 months after your HMSA coverage ends. To request a certificate, contact HMSA Customer Relations.
