Changing your health center

HMSA will review your request to change your health center on a case-by-case basis. HMSA may postpone your request if:
- You are an inpatient in a hospital, skilled nursing facility, or other medical facility at the time of your request.
- The change could have an adverse effect on the quality of your health care.
- You are an organ transplant candidate.
- You have an unstable, acute condition for which you are receiving active medical care.
Effective date for health center changes
If your request to change your health center is received by HMSA on the 1st through the 5th of the month, you may choose either the 1st of the current month or the 1st of the following month as the effective date.
If your request is received on the 6th through the 31st of the month, the earliest effective date will be the 1st of the following month.
Plan dependents age 18 or older must contact HMSA directly to change their health center. Health center changes for minor dependents can only be made by the plan subscriber.
